Week 1 Challenge: Set Goals

It is important to understand that your diabetes or heart disease is a serious problem. Unless you take care of your body, your condition can cause more problems in the future. If you think about these future problems, you will be more likely to make good choices about your condition now.  Making good choices about your health is called "managing your condition."  Managing your condition involves making lifestyle choices and using prescribed medical treatments to be as healthy as possible. You can take steps to control the negative effects of diabetes and heart disease.

The first step is to set goals.  Setting goals gives you direction.  They help you to focus on your target.  Use this article to learn how to set goals.  Then use the worksheet below to set your personal goals.
